A PROFESSIONAL CREDENTIALS VERIFICATION CORPORATION

 

 

PRIMARY SOURCE VERIFICATION (PSV)

As used herein, Primary Source is one that possesses direct knowledge of the validity or authenticity of an Applicant’s credentials.

Custom Credentialing Services (CCS) will provide primary source verification and file preparation for all the elements required to meet the Joint Commission for the Accreditation of Healthcare Organizations (JCAHO) and other regulatory requirements.  The purpose of the Credentialing process is to ensure that a healthcare practitioner who wishes to provide specified levels of patient care within a hospital, clinic or other healthcare facility is qualified and competent.  Verification of professional credentials is the recognition of professional and technical competence.  The process includes the gathering of detailed information from Primary Sources on Providers of patient care and the selection of these Providers for participation based on compliance with specific standards.

Collecting Primary Source information follows a very strict checklist on every file.  There are no short cuts and there are no other projects, jobs or duties that interfere with the concentration needed to efficiently process each file in the same manner.  Custom Credentialing Services will deliver to the Credentialing Department a complete new or renewal file ready for presentation to Client's Credentials Committee, Department Chair or other internal committee formed to make decisions.

Custom Credentialing Services has no authority to interpret or provide an opinion on information collected, and is not involved in the actual granting of privileges or any other internal function of the Client.

APPLICATION REQUIREMENTS

Custom Credentialing Services will conduct a thorough review of each application including but not limited to disciplinary actions, professional liability insurance, liability claims, licensure, professional references, current and past hospital and clinic affiliations, National Practitioner Data Bank queries (NPDB) if designated by client as agent) and health status.

Secondly, CCS requires all requested documents indicated on the cover page of the application.  When the requested documents are not available, we ask that the applicant provide details on a separate sheet, have "notarized" and attach.

Next, the application details specific instructions when more space is needed.  When there are “gaps” in the applicants hospital privileges/employment history, or if the employer is no longer in business we ask that the applicants provide detailed information on a separate sheet, have "notarized" and attach.

Thirdly, CCS compares verified information with our credentialing criteria.  Additionally, we document any discrepancies which will be flagged within the credentials file.

EDUCATION AND TRAINING

Primary Source Verification (PSV) will be obtained from accredited institutions.  In addition to dates of entry and graduation from an accredited school, we will also request a copy of the certified transcripts to show that the applicants satisfactorily completed medical/dental training requirements.  Education and Training documentation includes complete names, addresses and dates of ALL professional training schools attended including medical, internship, residency, fellowship, psychology, nursing, teaching appointments, etc.

PROFESSIONAL REFERENCES

A professional reference is someone who is in the same or related field.  The applicants must provide the names of three (3) professional references who have observed their clinical training.  The references will be requested to provide information regarding, but not limited to, the applicant’s medical knowledge, technical and clinical skills, clinical competence, professional behavior, ability to relate to others, rapport with patients, if the applicant has been the subject of repeated complaints by patients, coworkers and medical staff, appropriateness of the procedures the applicant has requested and overall performance.

TURN AROUND TIME

Custom Credentialing Services (CCS) is committed to delivery of completed credential files within 60 days of receipt of a properly filled out application (new or renewal) and ALL supporting documents.  However, in cases where a Primary Source does not respond, the Applicant does not cooperate, or for whatever other reasons, CCS will notify the Client at the 60 day point.  The Client will be given the option of receiving the incomplete file with noted discrepancies or have CCS continue to work the file.

CONFIDENTIALITY

Custom Credentialing Services maintains the highest level of confidentiality with all data obtained through the credentialing process.  Credentialing information received from Primary Sources will only be released to persons or organizations that the Applicants have  agreed to.  Regulatory bodies in the review or certification process may view Credentialing information obtained.
