.CORPORATE ORGANIZATION
Custom Credentialing Services,
(CCS) was formed to provide the following services: Provider Application
Completion and Professional Credentials Verification. The company was incorporated on
January 17, 1997, under laws of the State of
Colorado.

The President and Chief
Executive Officer holds a Bachelors Degree from Colorado Christian University
in Organizational Management and an Associate Degree from Arapahoe Community
College in Health Record Technology. She has many years of experience in
various hospital positions. Also, she is a member in good standing with the
Colorado Association of Medical Staff Services (CAMSS). As well, she is
a member in good standing with the National Association of Medical Staff Services
(NAMSS) since 1987. Her Credentialing
experience began in 1986 and she has expanded her knowledge by attending
courses, seminars and workshops. Additionally, she has attended several conferences
sponsored by the Joint Commission for the Accreditation of Healthcare
Organizations. In 1993, she attended her first Joint Commission Field
Education Program (Supporting Medical Staff Functions for Medical Staff
Services Professionals) in Oklahoma City, Oklahoma. In 2000, she
attended her first (Credentialing Resource Center Symposium) sponsored by the
Greeley Company in conjunction with NAMSS in Phoenix, Arizona. |

The Vice President of
Operations holds a Bachelor’s Degree from Colorado Christian University in
Computer Information Systems, a Master's Degree from Regis University in
Computer Information Systems Management and a Master’s Certificate from Oracle
Corporation in Data Base Administration. He has extensive computer systems
experience including analysis, applications design and development, programming,
project management, systems operations management, and technical support. As
part of his Master’s Project, he developed a custom application program designed
to track Allied Health Professionals and Physician Credentialing information.

CORPORATE LOCATION
Custom
Credentialing Services is organized in a formal manner similar to a number of
firms engaged in the “background check” industry. Completed (original
copies) files will be kept in locked cabinets at its facility located in
Thornton, Colorado. The computers, software, telephones, copiers, fax and
all other machinery and equipment are in place.
CONFIDENTIALITY
Custom Credentialing
Services maintains the highest level of confidentiality with all data obtained
through the Application Completion process as well as the Credentialing
Verification process.
